Our Story
Enhanced learning from unplanned events
Our vision is to enable organisations have clarity and predictive data that informs improvement to serious injury exposure at the frontline. Our early consulting work although impactful was limited in its ability to be scaled up and shared with clients. We felt that we needed to fast track sharing of our own models that we developed and applied over countless industrial projects.
Since 2020, we have undertaken independent research, applied our analysis method to over 10k incidents and developed an expert system based on a SaaS-platform for diagnostic projects.
In 2025 we will launch and share our SCALE(R) investigation app and share our IP through comprehensive accredited e-learning and coaching tools. Our vision is to help safety professionals, leaders and organisations to use our tools and reduce serious injuries so that they are few and far between

Our team
Our team has deep hands-on experience in almost all 'high hazard' verticals, working with organisations in sectors such as mining, utilities, manufacturing, agriculture, oil & gas, food processing, utilities. This team has helped many clients to determine their obstacles to operational reliability, and made important contributions to the development of strategy and the execution of strategic improvement plans.
Alex is a seasoned management consultant bringing more than two decades of expertise to Incident Analytics, having worked across a diverse array of high-risk industries in Europe, Asia, and Australia. His background spans downstream petrochemical, manufacturing, transport, oil & gas, agriculture, utilities and mining sectors. Alex has been exposed to complex organisations where he has spearheaded safety improvement initiatives that cover a broad spectrum—from serious injury prevention, leadership and governance to workforce development.
Alex holds a Bachelor of Engineering with Honours, Bachelor of Science, and an MBA from Monash University. His extensive educational background and international experience equip him with a unique perspective on safety and risk management, making him a key asset in driving Incident Analytics' mission forward and a positive impact on clients.
With a robust background in operations management, organisation development, HR management, leadership, and culture development, Warren Smith is dedicated to enhancing safety performance across industries. Over the past twenty years, he has worked with both large and medium-sized enterprises, guiding them towards pragmatic safety performance improvements.
Warren's expertise notably includes managing exposure to serious injuries and fatalities, leadership development, behaviour change, safety governance, systems alignment, and the exploration of human factors associated with error and reliability. His Bachelor of Arts degree underpins a deep understanding of the social aspects of workplace safety, complementing his technical skills to form a holistic approach to risk management.
David is a seasoned full-stack software engineer with more than twenty years of experience in the IT and software development industry. His career includes significant roles in developing software solutions for major legal and professional services firms on an international scale. In addition to his technical expertise, David brings entrepreneurial experience from his involvement as a director in three previously successful companies.
His dual expertise in technical and managerial aspects makes him ideally suited to oversee the development and scaling of the software and technical infrastructure for Incident Analytics. David's role is crucial in ensuring that the company’s SaaS offerings are robust, scalable, and capable of supporting our clients’ needs from start-up to full commercial production.
Liz is an experienced senior Health and Safety Professional with a strong HR and Organisation Change background. She brings significant experience working in both senior and operational roles within work health and safety, organisation development, HR and quality management systems.
Liz’s consulting work within work health and safety encompasses a range of industry sectors including councils, aged care, manufacturing, engineering and construction, warehousing, logistics and agriculture.

SCALE® Analysis
Incident Analytics developed the SCALE® analysis technology to determine which incidents merit deep-dive analysis and better understand the people, operational, and system factors that contirnute to the conditions for unplanned events to occur. This methodology has been proven to more accurately determine precursors to serious incidents and improve control effectiveness
