Find out why your controls architecture may not protect your workers or critical business processes in the way they are intended, and how to address that

We review control architectures to assess the inherent design effectiveness. We identify any systemic weaknesses that exist such as vulnerability to human error, systems misalignment, or leadership and cultural influences.

The design of most critical controls must account for the influence of human factors such as the ability to notice change, memory, stress, fatigue, urgency, and the contributing organisational factors that exacerbate those human factors.

Find out which critical risk operating situations are least well-controlled, and which levers to pull that will ensure improved operational reliability.

High-risk tasks are not always set up for success, despite the well-intended efforts of leaders and their safety partners. Our analysis shows that the design of critical controls needs to be resilient to specific organisational and system influences.

We analyse records of unplanned operational events (incidents) to extract the key points of control failure, and the factors that are likely to contribute to perpetuation of those failures in the future.

Find out whether planned improvement actions will reduce exposure and the likelihood of repeat events, and what will strengthen actions moving forward.

Real risk reduction only occurs when plant, equipment, and methods of work are altered in a way that reduces the exposure to human error. Remedial action must fundamentally improve the risk profile of the task by adding additional layers of control or moving the existing controls up the ‘hierarchy’.

We analyse which actions are likely to produce meaningful reduction in exposure, and whether real learning is occurring as part of an organisation’s unplanned event investigation process.
